You do not need to send in a placement form.  Forms were sent home with children prior to vacation week. If you did not receive a 2009-2010 placement form and wish to provide information, please return a printed copy to the office by 

NO LATER THAN FRIDAY, MAY 1ST.

PARENT INFORMATION FORM FOR STUDENT PLACEMENT
 
There are three general areas listed below.  You may provide input on any or all of these topics.  This form will be shared with the placement team. If you have confidential information, please write a separate letter.  Mark the envelope 'confidential'.
 
·           Goals that you have for your son/daughter for the upcoming year.
 
·           Social Information:   Please share information about your child's social relationships. You may choose to mention friendships (please list at least three children, and we will do our best to place your child with at least one of the names listed) or other relationships you would like us to know about.
 
·           Student Learning Style:  Please describe how your child learns, his or her strengths, weaknesses, level of independence, etc.
