• CORI SORI Please bring this to the front office and be ready to show your license. This form must be submitted every three years. If you're unsure whether your CORI SORI has expired, please call the front desk at Ward to ask.

• Ward School Volunteer and Chaperone Safety and Confidentiality Agreement. Please bring this form to the front office or send it back to school in homework folders.
